1.Office hour?
Monday and Tuesday open 7:30 am to 4:30 pm. Wednesday, Thursday and Friday open 7:30 am to 3:30 pm.
Office will not answer phone During the Lunch time between 12:30 pm to 1:30 pm. Please leave a message during that time.
Office will be closed for the Holidays: New Year’s Day, Memorial Day, July 4th, Labor Day, Thanksgiving Day and Christmas day.
2.Do I need an appointment?
Yes, you need to make an appointment before you come. Please call us to schedule your appointment.
Specially, you need to contact us ASAP if you want early bird appointment. There’s a lot of patients who takes blood test early in the morning
3.I’d like to cancel my appointment or change it.
We value our patients and their busy schedules. As such, we have strict policies for no shows and same day cancellations. We require 24hours advanced notice of cancellation, to protect both our providers and to provide waitlisted patients the opportunity to book new openings. Except in cases of unavoidable emergencies, same day cancellations and no shows will be charged. Please notify us of a cancellation or reschedule in advance.
4. What if I’m sick before or after office hours?
If this is a life threatening emergency, Please call 9-1-1 or proceed to the nearest emergency-care facility. Otherwise, call us and leave a voicemail message.
5. What insurance do you accept?
We accept most major insurance plan such as Medicare, Medicaid, Aetna, Anthem, Carefirst, Cigna, Federal Employee Program, United Health and PHCS.
If you have HMO plans, please select Michael D. Kim as primary care physician.
We will help you present your claim, but you must take ultimate responsibility for your account. Most insurance plans have determined certain services that may not be covered, such as preventive visits, immunizations or elective services. We try to inform you of the rules ahead of time, but we don’t always know if a service is covered. Since there are so many coverage plans available, you are encouraged to review your policy and contact your insurance company.
6. What should I bring?
When you come to our office, please bring your driver’s license and insurance card. Please come to your first visit 30 minutes before it is scheduled to complete our medical history forms. Please bring in a bag with all of the medications you take including over-the-counter medications, vitamins and any herbal supplements you take. This will allow your doctor to get to know your medical history and to look for any dangerous interactions.
Bring any old medical or vaccination records you have. We will copy these for your records and give you back your copy.
7. What if I need referral.?
Please call us for the referral. In order for us to serve you better, we need Doctor’s name, address, telephone number, fax number. And please call us at least 2days before you need it.
8. What if I need prescription.?
You could get it when you visit. Please contact pharmacy if you need Refill and tell them to call our office. But if it’s more than 6 month, you need to re-visit our office.
9. What if I need a Medical record?
For your medical record, you must fill-out medical record release form and sign it. You could sign the medical record release form and send it to us by email or fax it.
10. When do I pay bill?
You need to pay due at time of service if you have co-pay or no insurance. If you have co-insurance or insurance deductible, we will be billing to your home after claim processed.
11. How can I pay bill?
We do not accept personal checks. You could pay bill by cash or Visa, Master and Discover card.
12. What about my blood test result?
We will call you within a week and give you the result.
13. What about Radiology test result?
After we received that result, we will call you. If you didn’t get that result within a week, Please call us back.
Shorter Wait!
At Fairfax Family Medicine Center, wait times are usually much shorter than in a hospital emergency department. Hospital-based emergency care is designed for rapid response to life-threatening situations. Priority is given to patients with the greatest need. Patients with less serious medical problems often must wait.

Primary care settings such as Fairfax Family Medicine Center have lower costs than hospital-based facilities. Therefore, your out-of-pocket expenses will be less and your care more affordable when you opt to receive medical services from us..
